Our TeamWinton Hills Medical & Health Center, Inc. dba WinMed Health Services is a primary health care Federally Qualified Health Center (FQHC) offering comprehensive medical care to persons of all ages. WinMed is a private non-profit organization dedicated especially to preventive health care or the diagnosis and treatment of minor problems before they become major problems. WinMed Health Services is sponsored by the Bureau of Primary Health Care (BPHC) and participates in the Health Disparities Collaboratives Chronic Care Model to promote healthy lifestyles and improve quality of life for the patients served. As a chronic care model, WinMed Care Teams utilized evidenced based guidelines: a patient registry is designed and patient self-management is emphasized. Community partnerships are also encouraged to help reach our goal of 100% access to quality health care and zero (0) health disparities. WinMed employs full time physicians, dentists, and other clinical support staff that provide services five days a week, including evening hours. In addition, services are provided to children in the Cincinnati Public Schools, Head Start, St. Bernard School District, public housing communities and residential communities. |
